Book a Session

Please note: Both Intake and Waiver forms must be completed prior to the start of the Initial Evaluation only (NOT Follow up sessions unless horse has not been seen by Cloudveil Equine Bodywork, LLC for 6mo). Owners are strongly encouraged to submit all paperwork before the start of the initial evaluation so that your horse’s session time isn’t spent on completing paperwork.

The intake form you fill out after selecting BOOK A SESSION provides a better understanding of your horse’s current physical and emotional status and recent care as well as prior injuries and/or experiences. This information will help guide the session for best results. You will fill out this intake form when you schedule your Initial Evaluation appointment below.

**Again, you do not have to fill out a waiver or intake form for follow up sessions, only Initial or Reevaluations.
